Medical Case Manager - Bilingual

AIDS Delaware’s mission is to support those with HIV/AIDS in Delaware and to prevent the spread of HIV through comprehensive services, education, and advocacy. The agency has an immediate opening for a bilingual (English & Spanish) Medical Case Manager. The successful candidate will work with clients to ensure adherence to medical regimens and achievement of health-related goals.

Essential duties include:

• Carrying a case load of approximately 50 clients
• Proper documentation of contact with client, referrals, etc.
• Completion of home visits, attendance at medical appointments, etc.
• Assisting with education sessions/events

 

Ideal candidates will posses:

• Bachelor’s degree in human services or social work - Required
• Bilingual (English & Spanish) - Required
• 1+ years of case management experience - Required
• Knowledge of best practices in social work - Preferred
• Knowledge of HIV/AIDS, current trends, affected populations, epidemiology, etc. - Preferred
• Strong communication skills - Required
• Proficiency with Microsoft Office applications - Required
• Valid driver's license and reliable transportation - Required
