POSITIONS AVAILABLE:

Case Manager

Duties: Case Managers carry a case load of up to 50 clients. Responsible for maintaining adequate contact with clients, assisting with medical adherence and overcoming barriers to that adherence. Often visits with clients in client's home and in medical settings - therefore, active driver's license, reliable transportation, and insurance are required. Ensures proper documentation of all services provided. Completes necessary assessments, with the assistance of an RN as appropriate.

Education/Experience: Bachelors Degree in Social Work or related field required, with at least one year of case management experience. Master’s Degree or certification as Registered Nurse or Licensed Practical Nurse preferred. Bilingual (English/Spanish) strongly preferred.

Required Skills:
Knowledge of or experience in interviewing and assessing clients
Knowledge of or experience in caseload management
Knowledge of resources available to clients
Ability to communicate effectively
Ability to work well with diverse populations
